Anecdote
I'm off to the USA for a week on Tuesday, and only just today remembered that I had to apply for authorisation under the Electronic System for Travel Authorization scheme. You're supposed to do it 72 hours in advance; I did it 70 hours in advance of my flight's departure, but fortunately that was still around 73 hours in advance of entering US airspace so I did it in the nick of time.
Airlines ought to mention these things to you. I know that it's an exaggeration to equate US border controllers with space aliens when it comes to a propensity to insert probes into unlikely bodily orifices, but I still don't want to annoy them...
